Services

Patient Services
- Immediately address any ongoing care issues at the bedside.
- Planning for discharge to home or another facility.
- Attend medical appointments to clarify treatment plans and assure each medical provider is on the same page.
- Home visits to assess and prepare for home discharge (equipment needs, safety issues, etc.).
- Medication review with physician(s) and pharmacist.
- Assistance in monitoring and following treatment plans.
- Medical bill review to verify accuracy and appropriate liability.
- Consultative assistance for patient experience issues including potential health disparities related to race and ethnicity.

Caregiver Services
- Bedside support while patient in hospital to clarify treatment plans, discharge plans, address confusion regarding care issues, assure all physicians/nursing on the same page.
- Assistance with planning for hospital discharge to home or another facility.
- Wellness visits to the patient’s home, facility, and/or office.
- Mediate family conflicts regarding care decisions.
- Assistance in monitoring and following treatment plans.

Healthcare Provider Services
- Ombudsman services for patient and/or caregivers in the facility or office visit; to address communication issues, care complaints, or billing issues.
- Assist with complex discharge planning needs to home or another facility.
- VIP care coordination.
- Electronic care management for contracted patients/caregivers.
- Mediate family conflicts regarding care decisions.
- Readmission prevention.
- Consultative assistance for patient experience issues including potential health disparities related to race and ethnicity.
